Making a pack would be a waste of time. Here you go some links:

That isn't icon set, but i used only few icons form this pack, and created shortcuts with custom icons, also changed folder icons.
GTK theme was Troll 2 with panel from KDE's kicker (included in azenis pack), but i don't have it anymore. Maybe you can find it at [link] by typing 'troll 2' or 'Kyle Carter' in search engine.
I know that you aren't satisfied and you expected well-prepared package with ready to use themes, but by making few reinstallations i lost it. Hope that you will be able to make your desktop similar. Cheers!
